Any changes or modifications to this document by the service requester would automatically result in this service contract becoming null & void

I (Service Requester), voluntarily sign this Release of Liability and assumption of risk in favor of Bharatiya Temple in consideration for the opportunity to use the Priestly Services offered by Bharatiya Temple located at 6850 N Adams Road, Troy MI 48098.
In consideration of the agreement signed by both parties, Service Requester, by signing below, herby waives and releases Bharatiya Temple, its Board of trustees, EC members, Employees, Agents and Volunteers from any liability and/or claim from cancellation of services, personal loss or injury or death that may arise from usage or non-usage of Priestly Services offered by Bharatiya Temple, Troy.
I hereby confirm that the number of attendees at all our private indoor/outdoor event or events would be in compliance with State(s) mandated guidelines & protocols.
I hereby confirm that a social distancing of 6 feet will be maintained by all the family members and attendees during the scheduled private event or events.
I hereby confirm that face masks will be worn by everyone (including fully vaccinated individuals) present at all the event or events except during consumption of food/beverages.
I hereby confirm that neither I nor any of my family members/guests will attend the event if they are experiencing COVID-19 relating symptoms on the day of the event or have tested positive for COVID-19 and/or had been in contact with people tested positive for COVID-19 within the last 14 days (effective as of the scheduled day of the private event).
I hereby confirm to inform the temple management if either I or any of my family members or guests/attendees of our private event do not meet any of the foregoing terms & conditions on the day of the event and is tested positive for COVID-19 within 14 days after the event.
I hereby confirm that the number of family members for the private puja conducted at the temple would be limited to 6 members.
I hereby fully understand that the foregoing terms and conditions regarding private priestly services can be modified and/or the services can be cancelled by the temple management at any time. Modifications and/or cancellation of services may be due to change in State(s) health guidelines / protocols, travel restrictions / mandate, priests’ personal contingencies etc.
I undersigned, acknowledge that I have carefully read the foregoing “Release of Liability and Assumption of Risk” document and understand its content. I am aware that by signing this agreement, I am waiving certain legal rights, including the right to sue the Bharatiya Temple for any reason.
